CASASTART Youth Case Manager

Range: $17.25-$19.16 per hour (Range 16)

Neighborhood House Programs
Since 1906, Neighborhood House has been helping diverse communities of people with limited resources attain their goals for self-sufficiency, financial independence, health and community building. Our services include early childhood education (including Head Start and Early Head Start), mentoring and tutoring, emergency referral services, self-sufficiency/employment classes and services (including ESL and citizenship), job placement and training, substance abuse services, transportation and outreach for the elderly and disabled and publication of the Voice newspaper. Annually, Neighborhood House serves over 13,500 individuals. Of those, 80 percent are refugees and immigrants.

Scope
CASASTART is a proven Youth Development Strategy that prevents substance abuse and builds communities. CASASTART was developed by the National Center on Addiction and Substance Abuse (CASA) at Columbia University. CASASTART is a community-based, school-centered program to keep high-risk youth (9-14 years old) free of drug and crime involvement. Using an intensive and coordinated marriage of schools, preventative services, and community-based law enforcement, CASASTART addresses the individual needs of participants as well as the broader problems of their families and communities. Neighborhood House has implemented the CASASTART model at two Seattle Public Middle Schools: Aki Kurose and Denny. Neighborhood House is looking to fill an existing position at part-time capacity for the remainder of the school year.

The CASASTART Youth Case Manager has primary responsibility to provide intensive case management services and coordinate a comprehensive menu of services for referred/eligible middle-school youth and their families, to prevent substance abuse, improve school performance, and build communities. These services include: determining eligibility, completing a needs assessment and case plan, making referrals and service linkages, counseling, performing home visits, crisis intervention, documentation of services, follow-up services, and evaluation. This person will take on the responsibilities for a group of youth already eligible and engaged in the program, making outreach and recruitment less critical than ongoing intensive case management services.

Responsibilities
  • Perform outreach and recruitment activities.
  • Coordinate intake and screenings to determine eligibility for referred youth.
  • Conduct a needs assessment and develop individualized case plans for each assigned youth.
  • Provide intensive case management services for assigned youth and their families.
  • Coordinate on-going services and new referrals with school staff and other service providers at bi-monthly Case Conferences.
  • Provide information, resources, referrals, and advocacy to link clients to appropriate services in a timely manner.
  • Provide counseling and crisis intervention services to referred youth and their families???
  • Make monthly home visits to provide and coordinate Family Services. Family services include: counseling, parenting skills training, stress management/coping skills, and addressing health or mental health issues including substance abuse.
  • Provide mentoring directly or through referrals to local organizations.
  • Provide follow up services which include support, monitoring, and crisis intervention???
  • Maintain good working relationships with school staff and other community service providers.
  • Assist with group activities, workshops, trainings, and special events (including summer events and activities).
  • Track and document all services and complete monthly reports.
  • Work as a member of the CASASTART team and the NH Education and Community services department.
  • Be available by cell phone on nights and weekends, as needed, for crisis intervention.
  • Perform other duties as assigned.

Minimum Qualifications
  • Bachelor’s Degree in Social Work or a related field. Relevant experience with youth and families may substitute for formal degree.
  • Two years experience working with youth and/or families in social work, human service, or school settings.
  • Outstanding interpersonal and relationship-building skills and a non-judgmental, positive, problem-solving attitude.
  • Team player who is resourceful, proactive and comfortable working with diverse populations.
  • Ability to communicate effectively, both orally and in writing, with individuals from a wide range of cultural backgrounds.
  • Ability to work with multiple partners and service providers.
  • Ability to work effectively in a multi-cultural environment in schools, family homes, and with co-workers.
  • Experience conducting group activities, trainings, and classes/workshops.
  • Computer literacy, including MS Office Suite.
  • Willingness to comply with established agency policies and performance standards, which may include productivity/personal responsibility, client/customer services, teamwork, and maintaining a professional demeanor.
  • Ability to work primarily during Public School hours and an occasional flexible schedule, including some evening and weekend.
  • Must pass the Washington State Patrol background check (re-checked every two years).
  • Must have a valid driver’s license, reliable, independent, transportation for frequent last-minute travel between Seattle worksites.
  • Current First Aid certificate (within 30 days of hire, maintain current certificate during employment).
  • Current CPR certificate ((within 30 days of hire, maintain current certificate during employment).
  • Child Abuse and Neglect Training (within 90 days of employment).
